Please complete the boxes below with the details of any disclosable pecuniary interests that you hold.

1. EMPLOYMENT, OFFICE, TRADE, PROFESSION OR VOCATION

Please describe in detail any employment, office, trade, profession or vocation carried on for profit or gain, together with the name and trading address of your employer and, if you are self-employed or own a business, the name of that company, partnership or other entity. You should include any public authority to which you are elected, appointed or co-opted and from which you are in receipt of an allowance.

3. CONTRACTS

Please disclose any contract for the provision of goods or services or the execution of works that is made between the Authority and you (or a relevant person or a body in which you or a relevant person has a legal and/or beneficial interest).

4. LAND

Please describe any beneficial interest in land within the New Forest National Park. If the land is not easily identifiable by a postal address, please provide further information, such as Ordnance Survey Co-ordinates, the Land Registry title or a location map.

6. CORPORATE TENANCIES

Please disclose below any tenancy where:

• (a) the landlord is the Authority; and
• (b) the tenant is a body in which you or a relevant person has a beneficial interest.

7. SECURITIES

Please describe below any beneficial interest in securities of a body where:

• (a) you are aware that that body has a place of business or land in the New Forest National Park; and
• (b) either:
  1. the total nominal value of the securities exceeds £25,000 or one hundredth of the total issued share capital of that body; or
  2. if the share capital of that body is of more than one class, the total nominal value of the shares of any one class in which you or a relevant person has a beneficial interest exceeds one hundredth of the total issued share capital of that class.

PART 2: OTHER INTERESTS AND DISCLOSURES

Please disclose:

• (a) any membership, or position of general control or management, of
  1. any body to which the Authority has appointed or nominated you;
  2. any body exercising functions of a public nature;
  3. any body directed to charitable purposes;
  4. any political party;
  5. any body one of whose principal purposes includes the influence of public opinion or policy; and
• (b) any other interest that you consider may be perceived to influence your decision making as a Member of the Authority, including without limitation the receipt of gifts or hospitality, all other business interests, employments, directorships, appointments or positions, all receipts of grant or other funding, all land outside the National Park, etc.

Please also include any matter that relates to a family member or other close associate(s)

GUIDANCE FOR COMPLETING THIS FORM

When to declare

You must declare your interests as set out below:

• within 28 days of becoming a Member;
• within 28 days of any change to your interests or a new interest arising;
• where the interest is relevant to a particular matter that the Authority is dealing with, including at meetings of the Authority and its Committees. You will also need to consider, under these circumstances, whether a Part 2B interest amounts to being prejudicial within the meaning of the Code of Conduct, and whether you consider you may be perceived to have predetermined an item of business;
• where an interest is declared at a meeting please also notify the Monitoring Officer within 28 days of that meeting;
• in addition to completing this form, please expressly inform the Monitoring Officer of any planning application that affects you personally, because you are the owner or applicant or otherwise; and
• we also ask that you annually review and, if necessary, update your form.

Please note that failure to declare a Disclosable Pecuniary Interest correctly or on time and certain other failures to comply with the provisions of the Localism Act 2011 amount to a criminal offence.

What to declare

Please describe your interests in as much detail as possible, so that a member of the public could easily understand the nature of the interest.

Please disclose all memberships at any tier of local government, including your appointing authority if any, and also including all Parish Council memberships and Parish Council Planning Committee memberships and particular positions of responsibility.

Please read the NPA’s Code of Conduct and Local Protocols before completing this form. Please seek advice from the Authority’s Monitoring Officer if you need help in completing this form and/or in seeking a dispensation under the Localism Act 2011 in respect of a DPI and/or where you consider an interest amounts to a ‘sensitive interest’.
